Description
Dementia Partners has been a support organisation for older people with dementia for over 5 years. More recently, we provided Care Quality Commission’s Experts by Experience programme, for 18 months, in Greater London and South England. We now provide affordable, exceptional and bespoke housing combined with specialised housing support, for people with dementia. This is cheaper than a care home and this extra care shared housing enables people to live in a familiar community home environment.
AFFORDABLE RETIREMENT HOUSING, IN A DEMENTIA FRIENDLY ENVIRONMENT
With economies of scale, we provide housing that is much more affordable than a care home, saving around 50% on the cost of a care home, offering room rentals at market rates + service fee. Our bespoke housing is for people with cognitive and/or sensory impairment. We provide a dementia friendly environment and person centred housing support, every day. We are first point of contact, for 24 hour Telecare, any paid carers and families.
Dementia Partners Additional Support Housing is offering affordable, exceptional and bespoke extra support. We are HMO compliant housing which when combined with our specialised housing support makes it possible for people with dementia to remain in a community home.
HOUSING AND SPECIALISED SUPPORT OFFERED
• Dementia friendly housing adaptations and
environment
• Three showers and toilets between 6 residents
• Homely and grand period home with dementia
friendly interior design and adaptations
• Spouses and family guests may sleep overnight,
in double rooms
• A local park and local eat outs to visit with
family or paid carers
• Cleaning, hygiene and laundry services included
in the service fee
• Lounge, outhouse, and garden communal areas
• Continuous fire safety monitoring
• More affordable option compared to a care home
OUR HOUSING SUPPORT MANAGERS:
• Are fully trained and on-site daily
• Are DBS checked
• Manage the diary for visitors who stay
overnight
• Maintain the property and manage utility
charges
• Maintain the fire alarm system, ensuring
obstacle free exit routes
• Assist carers and family with matters
related to supporting residents
• Facilitate and coordinate daily
activities
• Update residents with appointments and
assist with housing related paperwork
• Update the family weekly via email or
telephone call
• Work closely with paid carers to ensure
the highest quality of support for
residents
• Are responsive to feedback of residents’
and their families’ experiences
• Make your housing a positive experience
